DSM-5横断的症状モデルは、精神科診断に次元的、診断横断的な評価アプローチを導入します。レベル1尺度は、カテゴリー的診断を横断する13の症状領域(うつ病、不安、怒り、躁、身体症状、自殺念慮、精神病、睡眠、記憶、反復行動、解離、パーソナリティ機能、物質使用)を網羅する23項目の自己報告スクリーナーです。従来のカテゴリー的診断を次元的重症度評定で補完するため、DSM-5フィールドトライアルの一環として開発されました。

DASS-21やPHQ-9などのDSM-5準拠のスクリーニング器具で使用される診断横断的症状領域(うつ病、不安、ストレス関連苦痛など)。

In the DSM-5 cross-cutting symptom framework, depression is assessed as a transdiagnostic symptom dimension that screens for the presence and severity of depressed mood and diminished interest or pleasure across psychiatric disorders, regardless of primary diagnosis (APA, 2013). As a dimensional measure, it captures the frequency of depressive symptoms over a two-week period to supplement categorical diagnosis and inform clinical decision-making about co-occurring symptomatology.

不安

In the DSM-5 cross-cutting symptom framework, anxiety is assessed as a transdiagnostic symptom dimension that screens for the presence and severity of worry, nervousness, and anxious apprehension across psychiatric disorders, regardless of primary diagnosis (APA, 2013). It captures the frequency of anxiety-related symptoms to identify clinically relevant anxious distress that may co-occur with any primary condition and warrant further evaluation.

ストレス

In the DSM-5 cross-cutting symptom framework, stress-related distress is assessed as a transdiagnostic symptom dimension capturing perceived psychological burden, tension, and difficulty coping, screened alongside other domains to provide a comprehensive picture of psychiatric symptomatology beyond the primary diagnosis (APA, 2013). This dimensional approach recognizes that clinically significant stress responses frequently co-occur with and modulate the course of diverse mental disorders.

実践的応用

レベル1尺度は、さらなる評価が必要な精神科症状領域を特定するための臨床スクリーニングツールとして機能し、臨床医が併存疾患を検出し、経時的な症状変化を追跡するのに役立ちます。APAから無料で提供されており、簡易トリアージ器具としてルーチンの臨床環境で実施できます。レベル2尺度は、レベル1スクリーニングで指摘された特定の領域のより深い評価を提供します。

測定方法

DSM-5フィールドトライアルは、米国とカナダの11の学術医療センター(成人7、小児4施設)にわたるテスト-再テスト信頼性デザインを使用し、ほとんどの領域で概ね良好から優秀な級内相関係数を示しました。

批評と制限事項

潜在因子構造は実際の精神科集団においてまだ十分に調査されておらず、公開された因子分析研究はわずかであり、次元性に関するコンセンサスはありません。推奨される閾値は高い感度を示しますが特異度は低く(0.60-0.73の範囲)、診断精度が制限されます。臨床実践への採用は遅く、西洋以外の環境での文化横断的妥当性検証は限られており、この尺度の心理測定学的特性に関するメタ分析は公表されていません。
